I authorize Arco Excavation & Paving, Inc., (Arco), (the Company), to make such investigations and inquiries of all statements contained in this application. I hereby give the Company permission to contact schools, previous employers (unless otherwise indicated), references, and others, for the purpose of verifying performance practices such as safety history as required by 49 CFR 391.23(d) and (e). I hereby release the Company, employers, and schools from any liability as a result of such contact or responses to inquiries. In the event of employment, I understand that false or misleading information given in my application or interview(s) may be cause for dismissal at any time without previous notice.
